Report a concern anonymously

What you can report

Suspected violations of Ashlee's Code of Conduct or other policies.

Suspected fraud, conflicts of interest, or financial misconduct.

Security incidents, policy violations, or observed weaknesses.

Privacy, HIPAA, or PHIPA concerns, including suspected inappropriate use or disclosure of protected health information.

Discrimination, harassment, retaliation, or other legal or ethical concerns.

How anonymity works

The reporting form does not collect email addresses and does not require a Google account or Ashlee login.

Leave optional identity fields blank if you want to remain anonymous. Do not include your name unless you want a follow-up.

Do not include passwords, health card numbers, or other unnecessary personal health information in a report.

Ashlee leadership reviews submissions confidentially. We do not use this channel to identify reporters except where required by law.

Non-retaliation

Ashlee does not retaliate against any employee, contractor, or other person who reports a concern in good faith, or who participates in an investigation.

Retaliation against a good-faith reporter is a policy violation and may result in discipline up to and including termination.

Good-faith whistleblower disclosures, and disclosures by a workforce member who is a crime victim, are not treated as HIPAA Privacy Rule violations where the applicable conditions are met.

What happens next

Reports are reviewed by Ashlee leadership (CEO and Engineering Director).

Matters that may be security or privacy incidents are also tracked in Ashlee's private incident repository with assigned owners and severity targets.

If you choose to leave optional contact information, we may follow up. Anonymous reports are still investigated to the extent the facts allow.
